Registration of new patients is usually done once you attend the clinic. However, if you wish to pre-register to save time during your visit you may print and complete the registration form (click here), and either bring it with you or fax it to the relevant clinic prior to your appointment.

If you are an existing patient and would like to update us with changes in your particulars, please download the change of particulars form (click here). You may fax or mail the completed form to us.

If you sending your child with an accompanying adult other than yourself, please download our consent form and have this presented at the clinic.

If you are departing from Singapore and would like to request for the release of your medical files, please download our release of patient medical information form and present it at the clinic.
